How is IP reputation measured here?

From publicly-measurable signals: whether the IP is on major blacklists and whether it has forward-confirmed reverse DNS. The score is computed transparently from these — it is not a proprietary metric.

Why does reverse DNS matter?

Receiving servers distrust IPs without a PTR record, or whose PTR does not point back to the IP. Forward-confirmed reverse DNS is a basic trust signal for sending mail.

Can I check a domain instead of an IP?

Yes. The tool resolves the domain's sending IP (A record, then MX) and assesses that IP.

How do I improve a poor IP reputation?

Get delisted from any blacklists after fixing the cause, set a matching PTR record, and authenticate mail with SPF, DKIM and DMARC.

Is this the same as Sender Score?

No. Validity's Sender Score is a proprietary metric from their network. This is an independent, transparent score from signals anyone can verify.
